How Do I Know if I am Eligible for Hospice Care?

Determining eligibility for hospice services is a collaborative process. Patients who have been diagnosed with a life-limiting illness and have a life expectancy of six months or less (if nature runs its course), as determined by their healthcare provider, may be eligible for hospice care. The Caring LIke Family Hospice Team works closely with your healthcare team to evaluate your specific circumstances, considering factors such as disease progression, symptoms, and your personal goals and wishes. Our goal is to provide support that is tailored to your unique needs.

Caring Like Family Hospice supports patients with various hospice-eligible diagnoses, including, but not limited to:

  • Advanced Cancers (e.g., Lung, Liver, CLL, Breast, Prostate, Pancreatic…)
  • Heart Failure
  •  Alzheimer’s Disease
  • Parkinson’s Disease
  • Amyotrophic Lateral Sclerosis (ALS/ Lou Gehrig’s disease)
  • End-stage Renal Disease
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Respiratory Failure
  • Multiple Sclerosis (MS)
  • Senile Degeneration of the Brain not Elsewhere Classified (primary), Dementia (secondary)

PLEASE NOTE: 

The following diagnoses are not hospice-eligible as primary/ terminal diagnoses:

  • Dementia Unspecified,
  • Failure to Thrive
  • Debility
