STEPS TO INITIALIZE HOME HEALTH CARE

  1. Get a doctor’s order. You will need a doctor’s order to start home health care. Your doctor or allowed practitioner can refer you to a home health agency.
  2. Contact  Caring Like Family: Our Home Health staff will run eligibility with your insurance to verify your home health benefits.
  3. Schedule an appointment. The home health agency will schedule an appointment to come to your home. They will talk to you about your needs and ask you some questions about your health.                                                                                                                                                Please note, a home health RN would first need to assess the patient to initialize care before other disciplines i.e. PT, ST, & OT make their visits.
  4. Meet with the home health staff. The home health staff will meet with you to discuss your care plan. They will also talk to your doctor or allowed practitioner about your care and keep your doctor updated about your progress.
  5. Receive care. The home health staff will provide you with the care that you need. They will come to your home as often as your doctor or allowed practitioner orders.

Additional things to keep in mind:

  • You may need to pay for home health care out-of-pocket, depending on your insurance coverage.
  • You should be home when the home health staff comes to visit.
  • You should be willing to work with the home health staff to develop a care plan that meets your needs.
  • The home health staff will tell you the number of visits required for your course of treatment.
  • Please keep in mind, our agency has no say on how many visits your insurance is willing to cover.
